Developing Vocational, Social, Educational, and Recreational Skills

Mission Statement

All adults served through Shares, Inc. shall be included in the Program Development process to promote Individualized Program Plans, which will develop their vocational, social, educational, and recreational skills to their fullest capabilities.

Purpose

Program Development will provide assessment, planning, training, and record keeping for adults served by Shares, Inc. to coordinate all facets of training available and develop each individual's skills to their fullest capabilities. This will be accomplished through the inclusion of the person served in all facets of Program Development.

Individual Program Plans (IPP)

Individual Program Plans are developed and written for every adult served by Shares, Inc. The IPP is developed with active involvement of the person served in conjunction with the Interdisciplinary Team, utilizing assessment data either collected during the prior year, or in the case of new referrals, utilizing assessment materials provided by the referring agency. The IPP contains the goals and objectives for training and the methods proposed to achieve those objectives. Goals and objectives are created based upon the person's identified needs and upon their interests and choices. The IPP is reviewed at least semi-annually in an interdisciplinary staff meeting.

For those persons in Work Evaluation or Employee Development Services (formerly Work Adjustment), the IPP is developed after completion of those services when a recommendation has been made to continue in programs offered by Shares, Inc.

Principles of the Individual Program Plan Process

The development of the Individual Program Plan will implement the needs and choices of the person served. The IPP should be designed to maximize vocational, social, educational, and recreational skills that will allow the person served to reach their full potential. The process should have the overriding goal of community integration to the fullest extent possible. At all times the dignity, safety, and rights of the person served should be considered and reflected in the methods used to achieve goals and objectives.
